The Money Follows the Person project, funded by a federal grant from the Centers for Medicare and Medicaid Services to the Texas Health & Human Services Commission, is focused on sustaining and disseminating effective practices from the Cognitive Adaptation Training (CAT) intervention to aid community transitions for nursing facility residents with mental health or substance use disorders. The Contractor will provide up to 40 hours of consultation on the Self-Directed Care (SDC) Project, including stakeholder presentations and development of recommendations for Medicaid benefits. The work includes extensive data collection and analysis to support the evaluation of the Money Follows the Person Demonstration Managed Care Organization Transition Specialist Pilot and other related activities. Additionally, the project will develop quality measures to monitor Medicaid’s shift towards community-based care and reduce inappropriate institutional placements of individuals with mental illness.
