The Texas Institute for Child and Family Wellbeing (TXICFW) has worked on this project since 2019. As part of this project, they conducted a needs assessment of case management for Children with Special Health Care Needs (CYSHCN) and developed a best practices case management practice model. Additionally, they have implemented an interactive online and in-person training of the case management practice model to DSHS and provided ongoing technical assistance and continuous quality improvement. They have developed a “train-the-trainer” curriculum and an evaluation plan for a process evaluation to understand implementation of the practice model.

In continuation of this project, TXICFW will continue to provide ongoing technical assistance in the implementation of the CSHCN case management practice model, conduct a process evaluation for implementation of the practice model across the state, make any needed updates for the practice model and/or training based on feedback, and develop a sustainability plan for the practice model and training for the conclusion of the contract period.
