1. Identify how traditional health care falls short of effectively coordinating care and addressing social needs.
2. Define social work’s contributions to achieving the Triple Aim of improved health outcomes, more efficient care, and better care experiences.
3. Describe interprofessional workforce development initiatives and innovative care models that provide whole-person care for populations across the life course, in particular older adults.
4. Outline the important connections between practice, research, education, and policy in order to sustain these innovative practices.
